Epinephrine Autoinjector	Form must be provided to the school principal and/or nurse before the student may possess and use an epinephrine autoinjector to treat anaphylaxis in school.
Health History	Required form to enroll a student in the Canfield Local School District.
Immunization Form	Required form to enroll a studnet in the district. Must be signed by parent/guardian and physician.
Registration Forms	Required forms to enroll a student the Canfield Local Schools. Must, also, complete the Health History and the Immunization Form
